Health system counseling and care coordination- Since our services won’t cover every health problem (emergency care, surgeries, etc.), we assist patients in navigating the complex healthcare system:
Delegating higher level care to the appropriate location (i.e., recommending an urgent care vs. the emergency room)
Referral to specialists when appropriate
Quick follow up with your doctor after a hospitalization or major illness
Pricing:
Age 0-17: $59/mo (children require at least 1 adult to be a patient)
Age 18-49: $95/mo
Age 50+: $115/mo
*Please note, there is a one-time $75 enrollment fee to gather records and build out a new patient chart.
We do accept the direct primary care affiliated insurance plan from Community Health Options in lieu of membership. We are unable to enroll patients with Medicare at this time.
What membership doesn't cover: emergency care, hospital stays, surgery, and specialist visits. We recommend pairing membership with insurance or a health share, and we'll help you find the right care when you need it.
